LIFT Mentorship Program – Mentor Application

Please read carefully before signing:

  I understand that if selected as a mentor, the information above will be shared with my mentee to facilitate a successful mentor/mentee relationship. I agree to hold harmless Suits, Stilettos and Lipstick (SSL) Foundation, its employees and board members from any and all claims, demands, liability, loss, damage and/or injury of any kind resulting from actions related to the LIFT mentorship program.  

Photograph & Video Release

I hereby grant permission to Suits, Stilettos and Lipstick Foundation and its affiliates to the rights of my image, likeness, and sound of my voice as recorded on audio photo or videotape without payment or any other consideration. I understand that my image may be edited, copied, exhibited, published or distributed for the educational, informational and marketing purposes and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording. I also understand that this material may be used within an unrestricted geographic area.
